Alameda County – Mentors on Discharge (MoD)

Connecting to the local mental health support community is one method to assist the patient as they transition from inpatient to less restrictive levels of care. This program asks the question will introducing patients, while hospitalized, to a peer mentor prior to discharge reduce the rate of re-hospitalizations as measured by length of time between hospitalizations?
